New Patient Registration
Who can register?
We are pleased to register anyone who lives within the practice area. A map of the area can be found on the above tab.
How to register
To complete registration with the practice each new patient will need to visit the surgery. You can print off our registration forms and complete them at home via the 'Registering Online' tab or you can complete the whole process in person at this initial visit. In both cases, you will be asked to sign the form to confirm that the details submitted are correct.
We require all new patients to show proof of residency on registering at the Practice eg. a utility bill, council tax letter or similar. If you have not been resident in the country previously we will need photographic identification.
When you register you will also be asked to fill out a medical questionnaire.
Please note, you need to complete one registration form and a questionnaire for each member of your household.
If you have just moved to the area and are living with family already allocated to a particular doctor you may request to go on the same doctor's list.
You can also book appointments and request repeat medication on-line. Speak to reception for more details.
Our reception team are very experienced and will ask you for brief details about your problem when you phone to book an appointment. Please see the attached poster that explains why our GPs require our reception team to ask you for these details Telephone Appointment Information
