Patient Group Registration'Pre-Registration' With The Practice


Whenever you register with a Medical Practice you are asked to fill in a medical questionnaire and other forms.  This is because it can take a considerable time for your new surgery to receive your medical records.

If you wish to complete these forms in preparation for your registration visit to the surgery, please print and fill out the registration form and relevant (dependent upon age) questionnaires, signed and fully completed and bring them to the surgery.

For those patients over the age of 40 we will ask for a Blood Pressure reading - this can be done in our waiting room PODs.

We require all new patients to show proof of residency on registering at the Practice eg. a utility bill, council tax letter or similar.  If you have not been resident in the country previously we will need photographic identification.
